COVID-19 INFORMATION PAGE
What is your COVID-19 Policy?
We are asking everyone to review and sign our main COVID-19 policy and adhere to this every treatment. We are sending this to everyone who does not have one on file via your patient portal so you can complete this electronically at home, prior to your appointment. If you cannot find yours, please reach out to your provider to help you via their email on the Team site.
If this is not possible, please see the below attachment for the required initial agreement prior to receiving care at physical therapy. Please complete this prior to your appointment and email it to us, or fill it out at home, email your physical therapist that you have it, consent and are bringing it with you, and sign it prior to your treatment session.
Following signature of this initial policy, our electronic medical records system (Kareo) will send you a symptom screening the day before every visit. If you have any of the symptoms on this form please cancel your appointment, stay at home, and contact your medical provider for further guidance. If you do not receive electronic forms, please call the office the day of your appointment to complete our screening, or email your provider for help. Please see the bottom of this web page for our symptom questionnaire.
What are your ongoing requirements to attend an appointment?
Every patient attending must be receiving essential care.
Every patient attending must complete the initial above COVID-19 policy agreement.
Prior to every visit, after signing this agreement, every patient will be asked to review questions 1-4 of that policy. To receive an additional appointment the answer to these questions must be no.
Every active patient should be utilizing their patient portal and electronic paperwork to allow touch free payments, scheduling, and registration or compliance with updated policies. This can be accessed from your phone or your computer. If you do not have an email address you will be required to verbalize these over the phone, then sign our forms in person in a private treatment room with your physical therapist to minimize wait room waiting time.
Patients are expected to use email@example.com or phone when possible. We will not be allowing waiting room traffic to schedule or cancel appointments.
Patients are required to attend their appointment alone. If your care is essential please contact your provider to determine a safe way to proceed and potentially schedule off hours.
Your care must be essential to your health and everyday abilities to function as determined by your provider. If you are not coming in for in person care at this time, you have the option to maintain your physical therapy care via Telehealth.

Will I be required to wear a mask?
All team members at N2 Physical Therapy will be wearing masks, abiding by our mask wearing policy, and at this time we are requiring all patients to wear masks. Please see the below order by the State of Colorado:
What are my options for continuing care if I am unable to attend or considered elective at this time?

Symptom Screening Required Prior to Every Appointment, the Day of that Appointment:
1. Have you had contact with anyone with a confirmed case of COVID-19?
2. Have you had a fever, cough, or difficulty breathing in the last 14 days?
3. Have you participated in a gathering of 10 or more people in the last 14 days? If yes, please explain to your provider and/or office manager to determine if we need to reschedule your appointment.
4. Have you had contact with anyone with symptoms of fever, cough, or difficulty breathing in the last 14 days?
